1. Acknowledgement of Risk

The Party Organiser understands that gymnastics and related activities involve inherent risks, including but not limited to falls, collisions, use of apparatus, and physical exertion, which may result in injury. By signing this agreement, the Party Organiser accepts these risks on behalf of all children attending the party.

2. Medical Information

The Party Organiser confirms that: Parents/guardians of attending children have provided consent for participation. Any relevant medical conditions, allergies, or additional needs have been disclosed to the Gymnastics Centre prior to the party. Required medication (e.g., inhalers, EpiPens) will be brought to the party and remain accessible. The Gymnastics Centre must be informed in advance of any child requiring special consideration.

3. Supervision Responsibility

The Party Organiser agrees that: Responsible adults will remain on site for the duration of the party. Children will follow all instructions given by Gymnastics Centre coaches and staff. The Party Organiser is responsible for the general behaviour of party guests outside of structured coach-led activities (e.g., food area, arrival/departure).

4. Safety & Conduct

All children must: Follow centre rules and staff instructions at all times. Wear appropriate clothing (no jewellery, hair tied back). Behave in a safe and respectful manner. The Gymnastics Centre reserves the right to remove any child whose behaviour presents a safety risk.

5. Emergency Medical Consent

In the event of an accident or medical emergency, the Party Organiser authorises the Gymnastics Centre to administer first aid and, if necessary, seek emergency medical treatment. A responsible adult must remain on site throughout the party.

6. Liability

To the fullest extent permitted by law, the Gymnastics Centre, its staff, coaches, and volunteers shall not be held liable for injury, loss, or damage arising from participation, except where caused by negligence.

7. Photography

I give permission for photographs/videos to be taken for promotional purposes. I do not give permission for photographs/videos to be taken.

Declaration

I confirm that I am the parent / guardian of the birthday child and the named organiser of this party. I have read and understood the terms of this agreement and accept them on behalf of all participants attending the party
